National

The RCHN Community Health Foundation seeks to identify and support a representative and committed group of health center grantees and partners that are planning or are engaged in local or state initiatives to engage in “health neighborhoods” that support systems for population health management, while also addressing local, upstream causes of health disparities. Community health centers, designated look-alikes and primary care associations are eligible to apply for this award. FUNDING | The Foundation expects to fund five awards under this program, with an approximate amount of $125,000 per award. DEADLINE | Letters of Interest are due 5:00 pm eastern time on Monday, July 24, 2017. Selected applicants will be invited to complete a full proposal. Award announcements are expected to be made in mid-October 2017.

The Agency for Healthcare Research and Quality is interested in studies that develop new health IT strategies for implementing existing PRO measures in ambulatory care environments, including studies that support the care of people with multiple chronic conditions. FUNDING | The total costs (direct and indirect) for a project awarded under this funding opportunity will not exceed $400,000 in any given year or $1.2 million for the entire three-year project period. DEADLINE | September 25, 2017, by 5:00 PM local time of applicant organization.

In select states, the Pew Charitable Trust's Health Impact Project offers funding to communities and organizations focused on addressing social determinants of health through health impact assessments, in order to reduce health disparities. FUNDING | The Health Impact Project will award grants of up to $100,000 for health impact assessments to be completed within 24 months. DEADLINE | Proposals must be submitted via the online application system by 8 p.m. ET, September 15, 2017.

Georgia

The Healthcare Georgia Foundation supports existing healthcare services and health promotion programs that intervene in prevalent and modifiable health disparities among Georgia’s vulnerable populations. FUNDING | Maximum of $50,000 awarded for a grant term of 12 months; 8-12 awards total. DEADLINE | Full applications must be submitted by August 3, 2017 by 3:00 PM EDT.

Idaho

The Rural Health Care Access Program helps rural Idaho communities improve access to primary medical and dental health care through grants to remove barriers that prevent people from obtaining healthcare, strengthen healthcare systems, and develop partnerships to better serve communities. FUNDING | Grants of up to $35,000 per year for a maximum of one year may be awarded to eligible entities serving areas designated as Health Professional Shortage Areas and Medically Underserved Areas. Grants must be focused on telehealth projects, community development projects, and/or financial support for qualified clinicians. DEADLINE | Applications must be received by the Bureau of Rural Health & Primary Care on or before Wednesday, August 30, 2017, 5:00 p.m. Mountain Time.

Massachusetts

The Blue Cross Blue Shield Foundation of Massachusetts supports projects designed to protect the availability of coverage and care for the most vulnerable people in the state, and to support linkages between social services and health care that help to improve outcomes for residents. FUNDING | One-year grants up to $50,000. DEADLINE | Full proposals must be submitted by August 18, 2017 to be notified of grant funding on October 13, 2017.

Missouri

The Missouri Foundation for Health invites organizations in eligible regions of Missouri to apply for funds to implement innovative projects aimed at strengthening public health infrastructure and supporting ideas that foster learning. Projects should focus on core components of public health infrastructure including workforce, data/information systems, and/or the capacity of local public health agencies to provide public health services, rather than focus on specific health issues. FUNDING | There is $3 million available for grants. A maximum request amount does not apply to this funding opportunity, but budget requests should be reasonable and appropriate for the proposed project. Applicants may apply for up to three years of funding. DEADLINE | The deadline to apply is August 14, 2017.

New Mexico

The Community Foundation of Southern New Mexico is offering grants to assist non-profit organizations in improving maternal and child health and wellness outcomes in southern New Mexico. Proposals may include one or more of the following: prevention/wellness, education, direct services, support for positive relationships, and social determinants of health. FUNDING | The maximum award amount will not exceed $15,000. The committee anticipates making up to 8 awards. DEADLINE | Proposals are due July 21st, 2017 by 5:00 p.m.

New York

The Jacob & Valeria Langeloth Foundation is interested in projects that seek to improve the physical and mental health of individuals involved in the criminal justice system, as well as projects that seek to end the cycle of violence at a community level using a public health approach. FUNDING | $100,000+ grants available over multiple years. DEADLINE | For projects to be funded in July 2018, registration must be received by October 9, 2017, Letters of Intent received by October 16, and proposals received by December 15.

Oregon

CareOregon's Community Benefit Program seeks projects interested in preventing, identifying and responding to children’s trauma (Adverse Childhood Experiences) to be prioritized in the Winter 2017 round of funding. CareOregon’s intention is to learn the best practices for building resilience in families with young children and fostering trauma-informed environments where children gather, including childcare centers and schools. FUNDING | $20,000-$80,000. DEADLINE | Fall 2017 applications will be accepted starting Friday, September 29 and ending at 11:59pm on Monday, October 23.

Texas

The Episcopal Health Foundation supports efforts to formally bring behavioral healthcare together with primary care in an integrated model and to support additional efforts to increase access to behavioral healthcare services. They are currently welcoming requests for programmatic and systemic interventions and for the planning and evaluation necessary to do this work successfully in Texas. FUNDING | Various amounts available. DEADLINE | A Letter of Inquiry is required and due by July 21, 2017. If your LOI is approved, you’ll be invited to complete the full application process. That completed application is due September 15, 2017 with funding decisions announced in December.

Wisconsin

The Wisconsin Partnership Program at the UW School of Medicine and Public Health is pleased to introduce two new community grant programs, both aimed at improving health equity in Wisconsin. The two new grant programs are designed specifically to support projects that improve health equity through a focus on the social determinants of health — the nonmedical factors such as income, employment, education, housing and the quality of the places where people live, work, learn and play. FUNDING | From $50,000 for two years, up to $300,000 over four years. DEADLINE | The earliest Letters of Interest are due April 14, 2017. The earliest applications are due October 2, 2017.

The Wisconsin Medical Society Foundation focuses on providing support for physician-led, community-based or statewide programs to improve health through education and outreach. FUNDING | From $2,500 to $10,000. DEADLINE | Applications are due by 4:30 p.m. Central Time on Friday, September 15.

Recently Funded Programs

National

The Sumner M. Redstone Global Center for Prevention and Wellness at George Washington University recently received an $800,000 grant from The Kresge Foundation for its Building Community Resilience initiative to align resources and systems to strengthen community resilience, including pediatric health care and local governments. Currently, the initiative consists of a learning collaborative with five pilot sites across the country: Cincinnati, OH; Dallas, TX; Portland, OR; Wilmington, DE; and Washington, DC.

Kentucky

The Humana Foundation provided $640,000 to four nonprofit organizations in the Knoxville area to improve the health and well-being of East Tennessee residents, including the Knoxville Area Project Access program assisting patients with navigating and obtaining health services and building a community-based system for identifying and addressing barriers to positive health care outcomes for low income, uninsured patients.

Texas

The Hogg Foundation for Mental Health awarded more than $1 million to two Texas medical schools and one nonprofit organization to strengthen integrated health care programs that take a coordinated approach to address mental health. Among these projects, the University of Incarnate Word School of Medicine is developing an integrated population health program utilizing a team of health care medical students and clinicians in clinical, home-based and school settings.
