At the core of every community’s health problems is the fact that our fragmented approach to the health and well-being of our communities just isn’t working. Population-based methods may work well in medicine, but the same cannot be said for the social determinants of health.
To make real improvements in community health, an integrated, community-level systematic approach is needed. In this model, healthcare must join the community – not take it over.
