Bill Siwicki from Healthcare IT News recently connected with our customer, Advanced Health, to discuss their innovative care coordination efforts utilizing the Activate Care platform.
Based in Oregon, Advanced Health is a coordinated care organization (CCO) that coordinates whole person care for members of their community. Connecting members to medical, dental, behavioral health, and substance use treatment is no small task, as there are moving parts that need to be tracked efficiently. This is where Activate Care comes in. Advanced Health chose our cloud-based technology platform for its community care coordination and referral management needs.
Advanced Health has been using Activate Care for six months now and currently has 200 care plans on the platform. They have also integrated with a local community partner, allowing for further collaboration in the community.
