Activate Care is featured in the Center for Health Care Strategies’ Digital Health Products for Complex Populations database, a new
inventory of health technologies that are designed to improve care, engage patients, and address the health and social needs of complex, high-cost populations.
This new database, made possible through Kaiser Permanente Community Health, is intended to support health care providers, payers, and others interested in improving the health and quality of care for complex, high-cost patients. Activate Care is one of only 19 vendors selected for inclusion in the initial database.
Since 1995, the Center for Health Care Strategies has partnered with nearly every state in the country to promote innovations in publicly financed health care, especially for individuals with complex, high-cost needs. Its work brings together state and federal agencies, health plans, providers, and community-based organizations to advance models of organizing, financing, and delivering health care services.
