Activate Care, Marin County Partner to Implement Regional Whole Person Care Program

Activate Care, a national provider of healthcare collaboration software, today announced a partnership with Marin County, California, to help implement their regional Whole Person Care program. Whole Person Care is the state of California’s cross-sectoral, interdepartmental, multi-disciplinary strategy to address the social determinants of health and increase data sharing and care coordination for Medicaid populations with complex health and social needs.

Whole Person Care is focused on a particularly vulnerable group of California Medicaid (Medi-Cal) beneficiaries who have been identified as high users of multiple systems in their county and continue to have poor health outcomes. The integrated Whole Person Care model is designed to reduce homelessness, increase initiation of alcohol and drug treatment, and decrease emergency room visits and hospitalizations.

Marin County has recruited a broad range of stakeholders committed to Whole Person Care, including the County’s departments of Behavioral Health and Recovery Services, Epidemiology, Social Services, Adult Protective Services, Emergency Services (EMS), and Criminal Justice, and many community agencies encompassing healthcare, behavioral health, and social services across the community.

ACT.md will serve as our collective collaboration hub.”

Marin County will use Activate Care for coordinated case management for complex patients, high medical and social services systems utilizers, and homeless or precariously-housed county residents. In Activate Care, care coordinators will identify patient needs through standardized screenings and assessments which will inform the creation of shared patient-centered care plans. Activate Care will also serve as a secure, community-based, social health information network with multi-directional information sharing and collaborative task management among Marin County’s many Whole Person Care partners.
