Last week, Activate Care had the pleasure of sponsoring and attending the 2022 RISE Summit on Social Determinants of Health. There were over 500 attendees live in Nashville and hours of great conversations with experts from around the country on how to move the space forward. Specifically, our team joined discussions focused around innovative care models to address SDoH, including the newly launched Path Assist offering that proactively addresses the effects of social determinants. Our team has pulled together 5 key takeaways from the Summit:
